I am an applicant to access the “Online Deposit Payment” and agree to be bound to Master Style Co.,Ltd. as follows:

Article 1. Using “Online Deposit Payment“

• 1.1   Applicant agrees to use the “Online Deposit Payment ” with Master Style Co.,Ltd.
• 1.2 Applicant agrees to comply with the regulations, include terms of deposit payment of buying surgery and treatment service from Master Style Co.,Ltd. strictly.
• 1.3   The applicant agrees to pay a deposit for the procedure (session/ transaction) not less than 5,000 THB (Five thousand Thai Baht) in order to booking the procedure appointment before the actual service date.
• 1.4  After completed “Online Deposit Payment” transaction. The money will be credited to the company’s account in the next 2 business days.
• 1.5  Once you completed the Deposit payment process. You need to print out the Deposit Payment confirmation and present to the clinic staff on the visiting date.

Masterpiece Hospital have right to keep the deposit (5,000 THB) as a fine, if the customer cancel the operative appointment or did not attend to get the service for any reason without prior notice. Except in case of force majeure, you cannot attend for a specified period of time. The Company will consider on a case by case basis.

You will be asked for a fine 100 Baht as a service fee for re-checking your Online Deposit Payment process. Because the company needs to re-audit your database to be the information that you choose to continue the procedure.

The Company have right to amend or change the terms and conditions of the “Online Deposit Payment” in all cases as the Company deems appropriate without prior notice. I have read this and accept all the Terms and Conditions
